Application procedures:

  1. Application form SFAA192(2012) and Guidance Notes for Application can be downloaded from http://www.sfaa.gov.hk/cef/forms.htm.

  2. Complete the application form SFAA192(2012). [Department of Industrial and Systems Engineering (ISE) will complete Part C of the form. You should bring along your completed application form and confirmation sheet of your subject enrollment to ISE General Office in person. If you are genuine need to send it by post, please send it together with your self-addressed return & stamped envelope (Attention: Miss Vivien Shek, Department of Industrial and Systems Engineering, The Hong Kong Polytechnic University, Hunghom, Kowloon).]

  3. Submit the form SFAA192(2012) to Office of CEF before the commencement of the course , you may refer the commencement date from the 'Academic calendar' under student handbook.
